How we can help you:

> Navigate the aged care and NDIS (National Disability Insurance Scheme) government systems.

> Sign up with a Home Care Package or NDIS provider of your choice.

> Support to apply for housing.

> Connect with support services in your community, such as health, mental health, housing and homelessness, drug and alcohol services, social and community groups.

Who is eligible?

To be eligible for our program, you may:

• Be aged 65 years+.

• Be aged 45+ for Aboriginal and Torres Strait Islander people.

• Be prematurely aged (50 – 64 years old)

• Have no carer or support person to help you find the services you need.

• Have difficulty communicating because of language or literacy problems.

• Need help to navigate My Aged Care or other Government services.

• Be in an unsafe situation or vulnerable if you do not receive services.

• Require housing, disability and homelessness support.
